The likelihood of benefit depends upon the location of the pain and the presence or absence of inflammation. pain control strategies and resources are listed in Appendix D.
Reduce Your Alcohol Consumption
After NSAIDs, alcohol is the drug most likely to destroy normal intestinal permeability. More than one glass of wine or beer is likely to be detrimental.94
Don't Rely on Medication to Reduce Stomach Acid
The body's first line of defense against intestinal infection is the acid produced by a healthy stomach. |
| People in pain, for example, tend to experience more pain when they fear that their pain control will be inadequate than when they believe that ample pain management is within their reach.6
When I was in general practice, a woman burst into my office, extremely agitated, complaining of severe headache.
"I have unbearable pain in my head," she cried. "It started three days ago and it's getting worse. Nothing relieves it. I took aspirin, Tylenol, my husband's Percodan. You have to do something for me right away, Doctor! |

Then, when the suggestion for pain control was given, the arm potential reversed just as it had in response to procaine. Conversely, when a control subject was asked in normal waking consciousness to concentrate forcefully on one arm, its sensitivity to pain increased, and the hand potential became more negative. We found we could use this difference to determine whether a person was really hypnotized or just cooperating. |

| In such cases lack of proper pain control can result in long-term chronic pain and may even slow down your recovery from the tissue damage causing the pain. That means you will heal more slowly if your pain isn't treated properly.
In the past, moderate to severe acute pain was treated with injections of narcotic medication that was given on an "as needed" basis, causing patients to go from acute pain to temporary relief with widely varying levels of pain in between. This pain increased and then abruptly decreased according to the dose and timing, under-treating pain for most of the time. |

In Kyoto, Japan, at the famous pain control Clinic, I watched acupuncture and moxibustion treatments, the latter using a small herbal cone placed on acupuncture spots and lighted like incense. With arthritics, moxibustion gave faster pain relief than our conventional therapeutic techniques.
One day, an ambulance brought a woman to the pain clinic with second degree steam burns on both arms. She screamed with the pain. The common procedure in America was to first inject narcotics to ease the pain, followed by tissue cooling and ointment. |

| Relaxation Techniques
Another area of alternative medicine has also proven very useful in pain control. Although once — and sometimes still — dismissed as mumbo-jumbo, relaxation techniques have proven themselves to be effective tools in reducing chronic pain. Some relaxation techniques appear to release endorphins; however their major benefit is in
\ muscle relaxation. Because stress increases muscle tension and worsens pain, relaxing the muscles reduces this problem. |

My pain control program was one of the first complementary medicine pain programs in America, and we achieved some extremely dramatic results, using such techniques as acupuncture and nutrition, in conjunction with conventional Western modalities. I loved this stage of my life. Helping people in pain was extremely rewarding to me.
As I directed the pain program, I became ever more aware of the terrible toll that the stress of chronic pain takes on mental function. |

HMP-33 can be taken alone, or along with NSAIDs for people who need additional pain control. There are no known side effects to HMP-33. It even seems to have a protective effect on the mucous membranes in the stomach, whereas NSAIDs can cause gastric irritation. Studies performed in Copenhagen show that most patients who use HMP-33 experience significant relief in symptoms. It takes about three weeks, however, to see a real improvement. Granted, NSAIDs may work faster, but they have potentially serious side effects and cannot be used by everyone. |

Codeine was used for pain control. Ultrasound to the fracture site and Full Body Connection were used 2 to 3 times a week by several home health caregivers.
A Healing Touch practitioner witnessed an unusual healing with her son who sustained a compression fracture of the back. The tips of the vertebrae at T-10, T-ll, and T-12 were knocked off and could be seen floating free from the spine on X ray. He was treated with a back brace, which he wore faithfully for 2 weeks, and pain medication, of which he took only one dose. |

HAZARDS OF OPIATE PAIN MANAGEMENT IN CANCER
The research showing that opiates interfere with immune function has led some investigators to raise some important questions about the use of morphine for pain control in cancer patients, suggesting a search for alternatives. This is an important issue that should at least be raised as part of obtaining informed consent prior to the use of opiates for cancer pain management.

This nationally recognized authority on feline nutrition, grooming, and behavior patterns actually had some worthwhile information on pain control.
Ms. Frazier's favorite nutrient is the mineral calcium. "Calcium calms nerves and raises the threshold for pain and stress," she wrote. She advises cat owners with felines in pain to have their veterinarians "prescribe more calcium" to relieve the excruciating agony. But "to absorb calcium the body needs vitamin D," she adds. |
| Researchers speculate that brain-based pain control takes place when people or pets act heroically; they seem to disregard their personal pain, for example, the injured athlete who stays in a game or an arthritic dog bounding into a burning house through an open door to save his master's small child.
The herbs and nutritional supplements mentioned in this section are at least a beginning or point at which to start relieving the pain of a poor suffering creature. |

The evidence also suggests that treatment with a gluten-free diet may offer dramatic improvements far beyond those offered by medications aimed at pain control or down-regulating the immune system. Dietary therapies, although more controversial, do not cause the often unpleasant, and sometimes very dangerous, side effects associated with drugs, especially those used for pain management.
A study on psoriasis (a very unpleasant autoimmune skin disease] serves as a model for connecting gluten with autoimmunity. |

| The ultimate control of psyche over soma demonstrates the priority of the conscious mind over physiologic processes such as immunity and pain control.128 This puts an enormous responsibility on the physician. He or she must take full account of a patient's mental and emotional states when treating chronic or life-threatening disease.

Identification of a biochemical mechanism for placebo analgesia has done more to change the image of placebo than any amount of arguing about the importance of beliefs and the mind. |

Psychotherapy as an approach to pain control "is based on the assumption that the perception of pain occurs within a personal and interpersonal context. The general assumption is that dealing with critical intra- and interpersonal issues will reduce the impact of pain."
2. Hypnosis is the oldest and most widely used approach. "Numerous clinical reports have appeared in the literature over the last three decades to support its efficacy in treating cancer pain. |
| This is important because, when a stressor induces pain control with an opioid peptide, the presence of that peptide more often than not may enhance tumor development, just as morphine may support tumor development. Shavit writes:
There is growing evidence implicating opiates in the regulation of the immune system. Opiate addicts are known to be highly susceptible to bacterial, viral and fungal infections and, in fact, to have deficits in immune function. Acute and chronic morphine administration in experimental animals and humans usually produces immunosuppression. . . . |
| But a patient whose enjoyment of life is being destroyed by pain may prefer a more active intervention in pain control.
Immediate relief of pain is the first goal of therapy, followed by ongoing control of pain for the rest of the patient's life. The ideal goal of this strategy—complete freedom from pain—is rarely possible, but pain can almost always be eased so that the patient can bear what was previously considered to be intolerable.
In planning the therapeutic strategy, it is necessary to pursue a series of objectives. |
| In their psychological study, the researchers found a very marked enhancement of quality of life and of pain control without the need for opiates, even in advanced cancer:
Psychological information was obtained from the patients present at the centre. ... [A] striking feature was the high degree of control the patients felt they had over their health and, perhaps as a consequence, their high ratings for mood and confidence. |

CONVENTIONAL THERAPY
Treatment usually consists of medical prescriptions for pain control and reduction of endometrial growth. Antiprostaglandin medicines, such as Indomethacin, Ibuprofen, and Naprosyn, are often given to reduce pain.
Another popular pharmaceutical, Danazol, is also used for this purpose, but in some women it does not provide total or lasting relief. Studies show Danazol to have a good effect after surgery to remove adhesions. After some of the adhesions have been removed, it may help an infertile woman become pregnant. |

Sunflower seeds are among the best sources of phenylalanine, a chemical involved in pain control. Studies suggest that phenylalanine helps reduce pain by inhibiting the breakdown of enkephalins, chemicals involved in pain perception. In studies with both humans and animals, phenylalanine makes acupuncture more effective at reducing pain. In laboratory rats, the chemical enhanced the effect of morphine and made it last longer.
If I were in pain, I'd eat a handful of sunflower seeds—I'm a habitual seed muncher anyhow—and use ground seeds in a poultice on painful areas. |

THE EXPERIENCE OF PAIN A psychological model
Pain is generally acknowledged to be a complex physiological/psychological phenomenon. It involves motivational and emotional components and conceptual interpretation, which may or may not have their basis in actual nociception. |
| The fact that large fibers are activated by pressure, touch, massage, and vibration suggests a mechanism for such pain control techniques as acupressure, acupuncture, and transcutaneous electrical nerve stimulation (TENS). Such stimulation apparently closes the spinal gate via the large-fiber system. Melzack & Casey4 expanded this theory by proposing the possibility of a higher level gate, in the reticular or limbic structures of the brain, that probably mediates the drive to escape from unpleasant stimuli. |
| A considerable body of literature supports its importance in pain control, since a state of lowered autonomic arousal is incompatible with anxiety. While progressive muscular relaxation, systematic desensitization, and autogenic training are all well-established physiological approaches to muscular relaxation, meditation traditions provide quicker methods to achieve what Benson49 has called, the "relaxation response". |

The Eicosinoid Messengers
Among the key players in promoting the "good" side of the yin and yang of health—of critical importance in getting that playing field level—are a group of body chemicals called the eicosinoids (pronounced: eye-kah'-sin-oyds), which influence and regulate a wide range of body functions, including blood pressure, the immune system, blood clotting, inflammation, and pain control. We can maintain a supply of these eicosinoids by eating certain kinds of polyunsaturated fats, the primary one being linoleic acid. |

Olness had a different idea, a mind/body approach to pain control known variously as visualization, guided imagery and self-hypnosis.
"My surgeon was understandably dubious," she recalls. He might have refused a nonphysician, but he knew of Dr. Olness's medical specialty. For more than 20 years, she'd practiced and taught hypnotherapy, and she was one of the field's leading researchers. He reluctantly agreed to allow her to use self-hypnosis—provided that an anesthesiologist was on hand with standard anesthetics just in case. |
